Safety and Quality Improvement System (SQIS) SQIS (Safety and Quality Information System) is an online incident reporting system and a non-punitive mechanism to notify UCI Health leadership of patient safety concerns in our hospital. Please report “near-misses” and “good catches,” as well as errors. The hospital wants and needs your reports to optimize patient safety and the quality of our working environment. SQIS can be easily found in the “Quick Links” menu bar on the right side of the UCIHS Intranet home page. The IR Huddle Team includes the Chief Medical Officer, Chief Nursing Officer, Chief Pharmacy Officer, Director for Critical Care Services, Director for Risk and Regulatory Affairs, and other quality and safety leaders. Please submit your incident reports so the hospital may continuously improve the quality of care that we provide to our patients and your work experience. Resident Competency List In order to optimize patient safety, the Joint Commission and ACGME CLER require that the hospital maintains an easily accessible database which identifies the exact patient care activities and procedures in which each individual resident/fellow may engage without direct supervision by an attending. The UCI Resident Competency Database, which can be found on the UCIHS Intranet home page in the Quick Links menu bar on the right, allows any UCI health care provider to search for an individual resident/fellow by name and produces a list of all procedures/skills in which the program has deemed that particular trainee competent. Program Directors are responsible for keeping this list updated. Please keep in mind that each patient situation varies in acuity and complexity; if there is ever any need for greater supervision, no one on the health care team should ever hesitate to immediately contact the supervising physician for guidance and assistance, regardless of day and time.
