To join lab research for Bio Sci 199 Packet B, students must receive two approval notices. The first approval is from Bio Sci, which allows students to register for labs. The second approval is from the School of Medicine Office of Research, which allows students to conduct research. The Office of Research provides BIO 199 Packet B students with approval after successful completion of the documents below: Department Orientation Record UC Irvine Healthcare - Confidentiality Agreement Form Bio Sci 199 Health Clearance (Vaccination Verification from UC Student Health Center) Copy of Health Insurance Card UC Learning Center (UCLC) – Annual Training Student UC Learning Center (UCLC) – Fire Safety Download Packet B Forms Bio Sci 199 FAQs I cannot receive a signature from my PI for the Department Orientation Record form. What should I do? Should your PI not be available to sign your Department Orientation Record Form (pg. 4 of Packet B), the lab manager can also provide the required signature. Can my doctor provide my health clearance? No, only a nurse at the Student Health Center can provide clearance. Do not submit any other medical forms to the Qualtrics survey besides the official clearance provided to you by the Student Health Center. How do I submit my immunization records to the Student Health Center? You can find the steps on Packet B, page 5 titled 'Bio 199 Health Clearance FAQ' or visit the Student Health Center website. The Student Health Center hasn’t sent me my results, and my deadline is approaching. What should I do? Refer to Packet B, page 6 on how to check the status of your clearance. If not available when submitting your Qualtrics survey, input all other Packet B documents and submit a placeholder in the vaccine verification section while waiting for your vaccine verification results. Do not submit any other medical forms to the Qualtrics survey besides the official clearance provided to you by the Student Health Center. Your health details are protected under HIPAA; only authorized personnel such as the Student Health Center should receive this information. I have questions regarding my vaccines, which vaccines do I need? Please refer to page 5 of Packet B titled 'Bio 199 Health Clearance FAQ.' Only communicate your medical questions or concerns to licensed medical professionals. I’m locked out/or can’t access my UCLC. What do I do? Refer to Packet B, page 8 titled 'UC Learning Center (UCLC) Training Instructions.' Students only get temporary access for at most two to three months. Note: Students & Guests cannot use the UCLC until they fill out the Student and Affiliate Access Request Form. Additionally, if you try logging in, and it says, your account is not active, simply fill out the form above. This form must be filled out to gain access to UCLC. After submitting the form, it will take Human Resources two business days to process and three to five business days to respond. I’m having trouble accessing a UCLC module. What do I do?
