TNT Primary Care Improving Health for Older Persons with Education (IHOPE) Fellowship

A flexible CME training program that equips clinicians to competently manage the complex medical, behavioral health, cognitive and social needs of older adults.

A nine-month CME certificate program equips clinicians with advanced skills in whole person geriatric care.

Training Experience

  • Primary Care & Multidisciplinary Clinicians: Internal Medicine, Family Medicine, Geriatrics, Psychiatry, Neurology, Emergency Medicine, Pharmacy, Nursing and Women's Health.  

 

Eligibility 

Open to primary care and multidisciplinary clinicians, including MDs, DOs, NPs, PAs, PharmDs.  

What You Will Learn 

Through this fellowship, participants gain practical skills to: 

  • Assessment & Diagnosis: Perform targeted integrated geriatric health and geriatric behavioral health assessments and accurately identify early symptoms of mild-to-moderate psychiatric and neurocognitive disorders (e.g., dementia).
     
  • Geriatric & Behavioral Health Care: Apply the 5Ms framework of geriatric medicine to diagnose and manage complex behavioral health conditions, trauma and substance use disorders (SUDs).
     
  • Evidence-Informed Therapies: Deliver targeted interventions using geriatric health, cognitive-behavioral therapy techniques, motivational interviewing and appropriate medications.
